- On this song Eminem recounts his descent into drug dependency, during which he gained weight, grew depressed and lost his creative spark. Em explained to the Observer Music Monthly May 2009: "Honestly, I never really put the mic down. The problem was, as I'm going to be explaining over and over again for a while, is that I had a pretty bad drug problem. I was messing with Valium, Vicodin, Ambien and anything to [help me to] sleep. Basically I'd take Vicodin to get me through my day."
- The murder of Eminem's best friend and D12 member, Proof, in April 2006 did nothing to help his downward spiral. On this track, the rapper mentions Proof by his birth name, DeShaun. Em raps that his mourning helped him spiral out of control. In the Relapse liner notes he pays tribute to his fallen friend, writing: "Proof, no matter how much time passes, not a day goes by that I don't think of you. If it weren't for you, I would not be where I am today, and we both know it. I tried to write a song for you, but nothing was good enough, so I'm dedicating them all to you - and you'd be happy to know that I spazzed out on 'em again! I know you wouldn't have it any other way! F--- 'em all - let's get 'em!"
- It was reported in late 2007 that Eminem was stricken with pneumonia. On this track, he clarifies the story saying the cause of his illness was his immune system being weakened by drugs. He raps: "That Christmas, you know that whole pneumonia thing. It was bologna, was it the methadone, ya think?"

This song is off of Eminem’s The Eminem Show. In it, he talks about his dealings with the law because of his weapons charges, and how he isn’t afraid to use a gun. Later, he talks about his greatness in the rap game.
This song is middle of the story started with The Kiss and concluding with “Say Goodbye To Hollywood”.

The Real Slim Shady!

- Eminem performed this at the 2000 MTV Video Music awards, in a very memorable performance. Trailed by a legion of lookalikes, Eminem entered from outside Radio City Music Hall and performed the song on the way in. The guys playing the doppelgängers all had to dye their hair Eminem-blond for the performance.
- Kathy Griffin is the voice at the beginning that announces, "Will the real Slim Shady please stand up?" She is in the video. Griffin was on the TV show Suddenly Susan.
- The chorus from this song was included at the end of Weird Al Yankovic's "Angry White Boy Polka" on his album Poodle Hat. (thanks, Steph - SoCal, CA)
- The judge who sentenced Eminem to a fine and community service for pointing a gun at the road manager of the Insane Clown Posse told him "Now is the time to please stand up," a reference to the line "Will the real Slim Shady please stand up."
- The Wisconsin radio station WZEE was fined $7,000 by the FCC for playing an unedited version of this song. The station claimed a technical malfunction caused the CD player to cue up the wrong track. The FCC also fined Colorado radio station KKMG $7,000 for playing the edited version of this, claiming the song was "patently offensive." The fines were rescinded after the rap industry made the case that banning this is a violation of free speech.
